PDS SOAR Services


SOAR Program – SSI/SSDI Outreach, Access, and Recovery

PDS is contracted through the MN Department of Human Services (DHS) to assist Minnesotans in accessing Supplemental Security Income (SSI) and Social Security Disability Income (SSDI).

Purpose of the SOAR Program

The SOAR Program (SSI/SSDI Outreach, Access, and Recovery) helps adults in Minnesota who are experiencing homelessness or are at risk of becoming homeless apply for Social Security disability benefits (SSI/SSDI).

PDS Certified SOAR Advocates offer free, personalized support throughout the entire application process. We simplify the process and guide you step-by-step through all requirements to improve your chances of approval.

Who is Eligible?

To qualify for SOAR services through PDS, you must:

  • Be currently homeless, at risk of homelessness, or living in permanent supportive housing

  • Have a documented physical or mental disability expected to last 1 year or longer

  • Be a Minnesota resident

  • Be between ages 18–64

What SOAR Services Include

Our trained and certified advocates provide intensive, hands-on assistance with every step of the disability benefits process.

What to Expect

  • Your advocate will stay in contact with you from start to finish

  • PDS will complete your full application, including the Medical Summary and Functional Report, using the nationally recognized SOAR model

  • Services are certified and monitored by the Minnesota Department of Human Services (DHS)

Application Support & Additional Services

  • Complete and submit initial SSI/SSDI applications

  • Assist with reconsideration and appeals, including Administrative Law Judge (ALJ) hearings

  • Maintain and maximize Social Security disability benefits

  • Provide warm hand-offs/referrals to external community resources
