Family Navigation helps families transition from homelessness into permanent supportive housing. Referred through Hennepin County’s Coordinated Entry System (CES), the program supports families experiencing homelessness and struggling with housing instability.
Families are matched to one of five housing options:
Minnehaha Townhomes
Hook & Ladder
Scattered Sites
Emergency Housing Vouchers
This program is for families who:
Have at least one minor child
Are staying in an emergency shelter, domestic violence shelter, or unsheltered in Hennepin County
Have at least one adult with a disabling condition
Have been referred by CES for permanent supportive housing
A Family Navigation Case Manager (CM) will start by learning about your background, goals, and housing needs through an initial assessment.
Help gather documents and support families with background checks and landlord debt issues
Expungement application assistance (if needed)
Assistance in securing housing and setting up a new home
Tenant rights education and lease review
Help with move-in costs, furnishings, and utilities
Address changes and transitional planning
PDS Case Managers provide continued care to help families succeed long-term.
Monthly check-ins and crisis support
Referrals for mental health, therapy, and family services
Help with housing paperwork and renewals
Assistance and support creating resumes, job applications, and setting up interviews
Serving adults with children, disabilities, and those experiencing long-term homelessness
