HSS is a Medical Assistance (MA) benefit that helps people with disabilities, mental illness, substance use disorders, or seniors live independently in the community.
1. Housing Consultation
(For people without a waiver or care coordinator)
Create a person-centered housing plan
Help with documents, provider selection, and service coordination
Provide information on rights, appeals, and other supports
2. Housing Transition
Help finding and applying for housing
Education on leases, budgeting, and moving
Support with deposits, landlord communication, and benefit applications
3. Housing Sustaining
Ongoing help to keep your housing
Support with rent issues, landlord concerns, utilities, and lease renewals
Coaching on safety, benefits, and connecting to community resources
You may qualify if you:
Are 18 or older
Receive Medical Assistance (MA)
Have a disability or disabling condition (mental illness, substance use, long-term injury, etc.)
Have housing instability, such as:
Homelessness
Risk of eviction
Living in shelters or temporary housing
Leaving or at risk of entering an institution
If you have a CSSP (Coordinated Services and Support Plan):
No extra paperwork needed
If you don’t have a waiver or CSSP, you’ll need:
MA proof (PMI number or MA card)
Assessment (MnCHOICES, PSN, or Coordinated Entry)
Disability verification (Medical Opinion, PSN, or SSI/SSDI)
